What to include
- the email address of the affected account;
- a concise description of the problem and the approximate time it occurred;
- whether the issue concerns the web, Android or iOS version;
- a screenshot only if it contains no patient data.
Do not send patient data by email. Do not include a PESEL number, full diagnosis or screenshots showing patient information.
Availability
Messages are accepted at any time. Responses are provided on working days as soon as reasonably possible. Security incidents and suspected unauthorized access should be reported immediately.
